1. Application form and ward recommendation as per prescribed format
2. Complete citizenship and copy of father and mother
3. Marriage registration proof of father and mother
4. Proof of relationship with father and mother
5. Original and copy of the child's birth registration
6. Proof of migration and proof of permanent residence in case of migrants
7. Office recommendation in case of employee family
8. 3 copies of passport size photograph of the child
9. If the minor's identity card is lost or falsified and a copy must be taken, the application with recommendations and proof of complete falsehood and fraud.
